Test Submission Form

This form is to be used when submitting exams and quizzes to Testing Services for students to use their testing accommodations approved by Disability Services. This form can be used in place of the Exam Coversheet typically sent with an exam to Testing Services.

This form is intended for submitting one test for multiple sections and students. Exams for multiple courses or multiple exams (like midterm and final) should not be submitted on the same form. Doing so could confuse directions for the testing conditions.
