Test Reservation Form

Please fill out the form below completely. The below information is what we need for scheduling exams for proctored exams through Testing Services. Inaccurate information will cause difficulties with scheduling your exams.

To ensure staffing and compliance with health and safety expectations all reservations must be made at least three (3) business days in advance. If your reservation is made less than three days in advance, Testing Services may have to outreach to your faculty to determine next steps.
